
MEMBERSHIP CRITERIA
Planned Living Assistance Network, Inc. is an organization assisting those who suffer from a persistent and severe mental illness. This is accomplished by the various and comprehensive services we provide. In order to ensure that this organization is the best option for your loved one, certain criteria must be in place. The following are general guidelines which we use to evaluate:
- Must have a diagnosis of Schizophrenia, Schizoaffective Disorder, Bipolar Disorder, or Major Depressive Disorder. Those who suffer from a developmental disorder such as Asperger’s will be considered, as long as the person has one of the above mentioned diagnoses, as well. We are unable to assist those with a diagnosis of mental retardation.
- Must be over the age of 18.
- Must be able to take care of own basic needs.
- No active illegal drug use or alcohol abuse.
- No history of inappropriate sexual conduct.
- No recent incidents of aggressive or violent behavior.
- Able to participate in group activities.
- Has a desire to improve his/her life and participate in the recovery from mental illness.
If you are interested in obtaining more information about PLAN, please call our office at 972-690-7526. We will ask you some brief questions and mail you an information packet. Following that, we will contact you to set up an intake appointment. During the intake, we will obtain important clinical data about the consumer. You will also have the opportunity to learn about PLAN and take a tour of our facilities. It will be required for the person suffering from mental illness to meet with our Director of Clinical Services, Rob Novick, LCSW, to be evaluated before being eligible to enroll in PLAN, which can be done during this initial meeting.
This is a list of helpful information to bring with you during your intake appointment:
- Psychiatric evaluations and psychological testing results
- Names and phone numbers of treating doctors, including psychiatrists and therapists
- List of medications
- Picture ID
- Social Security Card
- Insurance card, Medicare Card, Medicaid Form
- Names and numbers of emergency contacts
MEMBERSHIP LEVELS
Non-Professional
Friends – $75 annually. Friends value the work of PLAN and wish to support it, but do not wish to avail of its services. Friends are welcomed and encouraged to participate in PLAN events.
Sponsoring – $350 annually plus $100 activity fee. Sponsoring members may attend special activities and the annual membership meeting. Sponsoring members may enroll their family member who lives with a mental illness for psychotherapy and/or case management (these services are fee-based), socialization and educational programs.
Professional – $125 annually. Professionals, those individuals with an advanced degree and/or professional certification and work experience that benefits people who have mental illnesses. Professionals are welcomed and encouraged to participate in PLAN events.
